    （一）當事人是公民的，應當查明雙方的姓名、性別、年齡、籍貫、住址和聯系電話；當事人是單位的，應當標明單位的名稱、地址、聯系電話和法定代表人或者負責人的姓名。

    （1） If the party concerned is a citizen, the name, sex, age, native place, address and telephone number of both parties shall be ascertained; If the party is a unit, the name, address, contact telephone number and the name of the legal representative or person in charge of the unit shall be indicated.

    （二）證明民事法律關系發生、變更、消滅等事實的證據：如合同、協議、債權文書（IOU、IOU等）、收發憑證、信件等；

    （2） Evidence to prove the occurrence, change and elimination of civil legal relationship, such as contract, agreement, creditor's right document (IOU, IOU, etc.), receipt and delivery certificate, letter, etc;

    （三）確定債權、債務數額的證據；

    （四）證明訴訟主體資格的證據：證明原告資格的材料的原件和復印件，如居民身份證、戶口登記簿、護照、香港同胞和澳門同胞的回國證明、結婚證書等證據；企業為原告的，應當提交營業執照、工商登記證等材料的復印件；事業單位應當提交事業單位法人代碼證；原告為銀行、非銀行金融機構的，還應當提供金融許可證。證明被告主體資格的材料復印件，如營業執照、身份證、護照等；被告是在工商行政管理機關登記的企業的，應當提交最近的工商登記表。被告人姓名變更的，還應當提供變更登記表；

Copies of materials proving the defendant's subject qualification, such as business license, ID card, passport, etc; If the defendant is an enterprise registered with the administrative department for Industry and commerce, he shall submit the latest industrial and commercial registration form. If the name of the defendant is changed, a registration form for the change shall also be provided;
